6th Grade Students may participate in all sports except:

• Football

• Cheerleading

7th & 8th Grade Students may participate in all sports.

MCMS Complete Physical Packet (2026-2027)

The first six pages (Insurance Information, Consent Forms, and Health History) should be completed and signed by a parent/guardian.
The last two pages (Physical Examination Form and Medical Eligibility Form) should be completed by a doctor.

Physical packets can be picked up in any house office or downloaded and printed from the school’s website. Please contact the athletic department if you have any questions or need assistance.
