Athletic Forms
Effective immediately and continuing through the 2025–2026 school year, the DragonFly platform will no longer be utilized for the submission, management, or storage of athletic participation paperwork, including but not limited to physical examinations and parental consent forms. This decision results from ongoing technical challenges associated with the DragonFly system, which have impacted its reliability and usability.
Consequently, we are reverting back to a paper-based process for the submission of all necessary eligibility documentation. In order to participate in tryouts, practices, or competitive events, student-athletes must have a fully completed and approved physical packet on file with the athletic department.
We understand this change may present some inconvenience and appreciate your cooperation and patience as we implement this transition. Please be assured that the health and safety of our student-athletes continue to be our highest priority.
The first six pages (Insurance Information, Consent Forms, and Health History) should be completed and signed by a parent/guardian.
The last two pages (Physical Examination Form and Medical Eligibility Form) should be completed by a doctor.
Physical packets can be picked up in any house office or downloaded and printed from the school’s website. Please contact the athletic department if you have any questions or need assistance.
